Request to Present (Poster or Oral)Registrants desiring to present a poster or oral presentation should indicate the poster/
presentation title on their Registration Form. Guidelines for preparing your poster/presentation will
be sent to you when your Registration Form is received. Note, the number of oral presentations
will be limited. The deadline to submit your abstract is July, 2011.
Registration FeeThe registration fee is US $425 before April 19, 2011 and US $500 from April 20 to July 1, 2011.The fee includes a copy of the abstract book, opening barbeque, continental breakfasts, breaksand meals, and a Gala Banquet. The cost for a guest to attend the two evening meals and theGala Banquet is US $120.

AccommodationsThe Holiday Inn Grand Island Resort and Conference Center is Buffalo’s only Island ResortHotel. Located on the upper banks of the Niagara River, the hotel is just 15 minutes fromNiagara Falls, New York and Ontario, Canada. The hotel will provide participants with a specialgroup rate of $109 per night (plus state and local taxes).The group rate will extend from August 17 to August 23, 2011.The Group ID Code for the meeting is CHD. Planning Your Travel / Travel InformationThere will be a few talks on Friday, August 19th starting around 3 or 4 p.m. followed by a barbecue.
Saturday and Sunday will be full days of presentations.
The Gala Banquet (business casual, please no jeans or sneakers) will be on Sunday evening around 7:00/7:30 p.m.
Buffalo International Airport (airport code BUF) is the closest airport to fly into.
Once in Buffalo, AAA Taxi, Tours & Limousine is available to handle your ground transportation
to and from the hotel. You can contact them via e-mail (info@aaalimotour.com) or telephone
(716-550-0550). Have your flight information handy to give them and be sure you mention
Group ID Code CHD
so they know you are part of the meeting. Cost of transportation will
depend on how many individuals are traveling in the van with you.
